“Mr. Dave”

 

David passed away Monday, Sept. 29, 2008, surrounded by angels and family in the Jordan Valley Medical Center due to complications of ALS/Lou Gehrig’s Disease.

 

A loving father, son, brother, uncle, and fiancé, David blessed the world with his unforgettable sense of humor, his sharp wit, wisdom, leadership, and unmistakable smile. As a Gospel Doctrine teacher in the LDS Church and a dedicated career social worker, “Mr. Dave” made the world an easier, more compassionate place to live in for everyone that met him. Dave’s career and life’s work was spent working with at-risk children and youth.  At 43, Dave put the capstone on his career, serving as the CEO of Manatee Palm Youth Services in Bradenton, Florida, where his legacy as a compassionate human being was solidified in the hearts of his clients and staff. A lover of University of Utah football, chess, orange Gatorade, Chessmen cookies, and fine chocolate “vitamins,” as he referred to them.

 

Among those he said “Farewell” to on Earth are his son David Jr. and daughter-in-law, McKenzie; his father Gary; his siblings Mark (Kim), Paul (Channy), Darryl (Shanna), Shannon (Rodney) Hickman, Bruce (Amy), Jon (Lexi), and Laura (Charles) Allen; his many loving nieces and nephews who live for “Uncle Dave”; and the love of his life, his fiancée, Tammy Squires.  Dave is preceded in death by his mother Sharon May; grandparents; and Uncle Keith.

 

A private family viewing will take place on Friday, Oct. 3rd at 9:30 a.m., followed by a public funeral service at 11 a.m. Both will be held at the LDS Thirty-First Ward Building, 1078 S. McClelland St. (1040 E.), Salt Lake City.  Interment to follow at Elysian Burial Gardens, 1075 E. 4580 S., Salt Lake City.  

 In lieu of flowers, there is an account at Wells Fargo Bank in the name of Gary W. Guymon for the benefit of the David W. Guymon Memorial Fund, which will be used for scholarships for at-risk youth—GO UTES.

Dave, your presence here is very missed; it is like a part of our family is missing. I personally miss getting a chocolate "vitamin" out of your office and our brief chats. I was saddened to hear of Dave's passing. I have fond memories of Dave at Utah State and the FIJI house and am sorry to have lost touch. It is good to see he accomplished much of value.

I have a lot of good memories working with Dave at Benchmark as I benefitted from his clinical experiences and ideas. He was part of my initial interview and I remember being greeted by his smile and serious questions. Foremost I appreciated our friendly exchanges around Utah and BYU football. I won't forget finding my office decked out in red confetti and a sign that said, "Don't mess with Dave's Utes" after the 2005 overtime game. Even after the last two years he was always a great sport, especially when he would be welcomed with the Cougar Fight Song on his voicemail each morning during rivalry week even when he was in Florida. Dave, thanks for the fun and memories! May Dave's family and loved ones be blessed with comfort.

Tami, I was heartbroken to hear of Dave's passing. Dave was a person who made the everything around him brighter, and I truly feel that the world has lost someone great. Please accept my deepest condolences. The Utes had an incredible win last night, and I suspect they had a little help from a devoted fan.

My deepest sympathy goes out to Dave's family and friends. I consider myself fortunate to have known Dave over this past year and to have worked with him on his legacy of compassion and treatment for mentally ill children. He was an inspiration to me and all of those who knew him.

Just today, May 4th, almost six months later I was deeply saddened to hear of Dave's passing. Dave worked at our outpatient clinic in two episodes for about a total of two years. I appreciated Dave's wonderful wit and wisdom. I was so happy that he had found achieved the job he had always wanted in Florida and the last I heard he was happily battling issues problems and things there. To his family I can only express my condolences and thanks for producing such a wonderful person. His memory is in my heart.
