Michael Ray Koziar
February 1, 1952 - February 26, 2017
Michael Ray Koziar of Salt Lake City, Utah, passed away on February 26, 2017 from Progressive Supranuclear Palsy. Mike was 65 years old. He is survived by his wife Cindy; son Michael; and daughter Kelsey; his mother Adele Koziar; and two sisters LaRae Harper and Robin Stark.
Mike was a great guy who always did his best to be a good husband, a good father, and a good friend. He will be greatly missed. No services will be held. Instead, we are all picturing Mike playing the golf course at Augusta National. In lieu of flowers, please make a tribute donation at www.psp.org in the hope that someday a cure can be found for this awful disease.
Mike and his family would like to thank the doctors, nurses and support staff at the George E. Wahlen VA Medical Center for the quality, compassionate care they provided him. You are appreciated.
